Best Practice Examples

As part of its Department of Health-funded Primary Care and Community Links project, Carers Trust have undertaken a scoping exercise across their networks to identify examples of best practice between carers’ organisation staff and GP Practices.

These examples of best practice focus on engaging GP Practice staff, and working with them to increase the identification of carers and their referral to support services.
